Performance appraisal attempts to achieve three objectives:
- Obtain agreement between the individual and his/her appraiser on the job to be done and the results to be accomplished during the coming year.
- Indentify training and/or guidance to improve present performance and achieve greater capability in the future.
Provide an apportunity for a mutual exchange of views on the individual's past performance.
The reason for a formal procedure is to produce is to produce a degree of consistency between different appraisers
and to ensure that the appraisal is carried out as fairly and fully as possible.
Before your appraisal interview, please complete this questionnaire. If includes a list of statements which may be
applied to your performance in your current pots, rate yourself as accurately and honestly as you can. Answering these
questions in advance will enable you to clarify your thoughts and highlight the most pertinent issues, these can then be
discussed at the apraisal interview with your appraiser.
This form is your property, but let your appraiser have a copy five working days before your interview. If will only be
attached to the official form if you expressly request so.
The final appraisal form will be completed by your manager, or his/her nominated representative.
However you will be given the opportunity to comment on it, and sing the final document.
